Your liver does not complain. That is the whole problem.
It has no meaningful pain fibers. It can be carrying a significant load of stored fat and still show up on a basic panel looking unremarkable. So it keeps working — processing fats, helping manage blood sugar, clearing what your body needs to clear — under more and more strain, quietly, for years.
But it does send signals. They are just easy to dismiss, because each one looks like something else. Being tired looks like being busy. Weight around the middle looks like turning 40. Hormone symptoms get handled one at a time by different specialists, and nobody stands back to read the pattern.
Here are the five signs of fatty liver that get missed most often, and what the research says each one is pointing at.
How common fatty liver actually is
A 2025 review in Clinical and Molecular Hepatology put the global prevalence of metabolic dysfunction-associated steatotic liver disease at roughly 38% of adults — an increase of about 50% since the 1990–2006 period, with a projection of 55.4% by 2040.1
This is not a rare condition you are unlikely to have. It is closer to a coin flip trending the wrong way, which is exactly why the signals are worth learning.
Sign 1: Fatigue that rest does not fix
Not the tired a good weekend resolves. The kind that is there when you wake up, sits behind your eyes through the afternoon, and does not respond to more sleep or more coffee.
The mechanism is unglamorous. A liver under strain does not process nutrients efficiently, so you can eat well and still not get full use of what you ate.2 The energy you are looking for is upstream of the food.
What makes this one slippery is how reasonable every other explanation sounds. Age. Stress. Poor sleep. All plausible — and none of them rule out the liver.
Sign 2: Midsection weight that resists everything
You have tightened the diet. You are moving more. The scale does not care.
Fatty liver frequently travels with insulin resistance, and that pairing is what makes abdominal fat behave differently from fat anywhere else.2 It is not a willpower problem and it is not a calorie-arithmetic problem. Two clues that point here rather than at your effort:
- The weight concentrates at the waist rather than distributing.
- The same approach that used to work — a cleaner month, a training block — produces less than it used to.
Sign 3: Fast food as a share of your calories
This is the one where the detail matters, because the popular version of the claim is wrong and the real finding is more useful.
Researchers at Keck Medicine of USC surveyed roughly 4,000 adults. About half ate some fast food. Liver fat did not rise across that whole group. It rose in the subset — 29% of the fast-food eaters — getting 20% or more of their daily calories from it, and the elevation was most pronounced in people with obesity or diabetes.3
So the question is not “did I eat fast food this week.” It is what share of what you eat comes from food you did not make. That reframe is more honest and more actionable:
- One meal out on a Friday is a different exposure than lunch out five days a week.
- Convenience food eaten at a desk counts, even when it never came from a drive-through.
- The 20% figure is a ratio, not a count — it moves when you change the denominator, not just the numerator.
Sign 4: Hormone symptoms that keep getting handled separately
This is the connection that surprises people most, and it is well documented.
Estrogen is sent to the liver to be broken down, then passed through the colon once it has done its job.4 That single fact reframes a lot of symptoms: if clearance is sluggish, the issue is not only how much is being produced, but what happens to it afterward.
Thyroid runs through the same territory. A 2025 review in Nature Reviews Gastroenterology & Hepatology notes that hypothyroidism is associated with metabolic dysfunction-associated steatohepatitis, and that reduced thyroid hormone concentrations in the liver raise the risk of steatotic liver disease. The liver is also where a meaningful share of T4 is converted into the more active T3.5
And insulin: research on the hepatic insulin receptor places insulin resistance in the liver at the center of how fatty liver develops, not off to one side of it.6

Gallstones. Right-side discomfort after fatty meals. A gallbladder that came out years ago and has not been mentioned since.
A Korean cohort study of 661,122 people compared 4,664 who had their gallbladder removed against 13,992 matched controls. The cholecystectomy group had a 1.48-fold higher risk of developing steatotic liver disease. In people with three or more cardiometabolic risk factors the hazard ratio climbed to 4.45, against 1.22 in those with fewer than three. Eight-year cumulative incidence reached 23.48% in the highest-risk group, versus 7.48% in the lowest.7
The authors' own conclusion is the useful part: comprehensive screening and management of those cardiometabolic factors matters both before and after the surgery. “The gallbladder is out, so that chapter is closed” does not match the data. What the research points to
The encouraging part of this literature is that liver fat is responsive. It is not a fixed diagnosis you carry unchanged. Diet and lifestyle intervention is the most consistently supported lever across the reviews, which is inconvenient, because it means the work is ongoing rather than one-time.
Where the evidence concentrates:
- Lower the share of ultra-processed and convenience food, rather than chasing one banned ingredient.
- Cut added sugars and high-fructose corn syrup, including where they hide — condiments, sauces, sweetened drinks, most packaged baked goods.
- Build meals around foods with a track record here: cruciferous vegetables, alliums, omega-3-rich fish, eggs, avocado, unsweetened coffee and green tea.
- Move. Physical activity shows benefit for liver fat somewhat independently of what the scale does.
- Support the pathways nutritionally, so the machinery has what it needs to run.
Two numbers worth asking about
Neither is on a standard panel by default, and both are ordinary requests:
- GGT — a liver enzyme often more informative in this context than AST and ALT alone.
- Fasting insulin — which can be elevated while fasting glucose and A1C still read as normal.
Bring both to your provider. You are asking for information, not a diagnosis, and a baseline makes anything you change afterward measurable.

Frequently asked questions
What are the earliest signs of fatty liver?
There is usually no pain, so the earliest signals are non-specific: fatigue that rest does not resolve, weight accumulating around the midsection, and hormone symptoms that get attributed to something else. Because none of these are unique to the liver, they are worth raising with a provider rather than self-diagnosing.
Can fatty liver be improved?
